Pursuant to Section 14 of the Act (820 ILCS 305/14), the Chairman evaluates Arbitrators seeking reappointment and may recommend them to the Governor.

To assist in this process, please complete a brief survey for any Arbitrator with whom you have had direct interaction. The survey is easy to use and includes space for comments.

Only Arbitrators seeking reappointment are included. Please do not submit evaluations for individuals you have not worked with.

Insurance Coverage

Illinois law requires most employers to provide workers' compensation insurance. The Employer Insurance Coverage Search web page provides information for

employers that have bought a wc insurance policy (based on the National Council on Compensation Insurance (NCCI) database); and
employers (parent companies and their subsidiaries) that have obtained IWCC approval to self-insure.

The database does not include Illinois employers that are in self-insurance pools, which are regulated by the Illinois Department of Insurance.
